Appropriate medical/clinical record documentation practices are an integral component of a practitioner's practice.

Documentation is also useful in avoiding, or in defense against, a malpractice claim. Insurance companies often review records to support the necessity of care, either concurrently or retrospectively.

The U.S. government CERT program found that chiropractor's notes were below standard and that:

• 83% had insufficient treatment plans.
• 72% no treatment plan.
• 77% of treatment plans had no goals or objective measures or progress reports.
• 66% did not check for contraindications.

What are the keys to good documentation?

Legibility tops the list, followed closely by consistency, completeness, accuracy, and timeliness. "Documentation needs to reflect what actually happened," says FP Edward Alan Hirsch, vice president of John Deere Health in Kingsport, TN.

And, as the adage goes, if it isn't documented, it isn't done. Notes Susan Reese, a coding consultant with MedaPhase in San Antonio, "You can swear you had a conversation with a patient, but few courts or chart auditors will accept your word if there's no entry to back it up."
